RN Case Manager
The registered nurse is in charge of planning, organizing and directing hospice care. This person has experience in nursing as well as community health education. In a job as an RN, you’ll use community resources to plan and direct services that meet the needs of patients and their families, whether in their homes or in their communities.
LPN CC, PRN
The licensed practical/vocational nurse works under the supervision of an RN to provide care directly to the patient. In this job, you will be responsible for following the plan of care and administering treatments, as well as collaborating with the Capital City Hospice care team to provide a positive care experience.

STNA Certified Hospice Aide
The STNA/hospice aide is a paraprofessional team member who works under the supervision of an RN and as part of an interdisciplinary group. In this role, you will provide patient services as they’re needed to promote a comfortable environment and meet the patient’s personal needs. Additionally, you will be responsible for observing the patient as well as documenting and reporting these observations and any performed care. When assigning this role, we consider what’s best for quality, continuity, and the safety of the patient’s care.
